The Self-Determination Program was signed into law by Governor Edmund G. Brown. SDP offers individuals and their families with more freedom, control and responsibility in choosing services and supports
The Five Principles of SDP
FREEDOM:
You have the right to plan your own life and make your own decisions.
AUTHORITY:
You decide how your budget is spent for your services and supports.
SUPPORT:
You select supports and people that help you live and work.
RESPONSIBILITY:
You make decisions in your life
CONFIRMATION:
You have the right to make decisions in your life by designing the services you rely on.
SDP PROCESS
COMPLETE ORIENTATION
The first step to the SDP is completing an orientation. Contact your Service Coordinator to learn more about how to attend an orientation.
COMPLETE A PERSON-CENTERED PLAN (Optional)
A Person-Centered Plan will help you:
-Identify your goals and dreams
-Identify what you like and interests
-Set meaningful goals for your life
-Choose who will provide services
-Updated annually
SELECT SUPPORTS
Once you have completed your PCP, Growth & Care can assist you transition into SDP. We assist in the following:
make informed decisions
Locate, access and coordinate the services in your IPP
Leading/participating and advocating on your behalf during the IPP and Budget Meeting
