Coordinated Family Supports (CFS)

CFS is a new program designed for adults who are 18 years and older who continue to live in their family home. Individuals must be served by a Regional Center.

CFS provides assistance and coordination of services and supports such as:

  1. Developing skills in the home

  2. Coordinating and getting services delivered, including help with “generic services”

  3. Helping people access their community

  4. Coordinating health/medical appointments

  5. Assisting with scheduling and referrals across agencies

  6. Help understand and navigate systems

  7. Help secure other services such as respite, transportation and resources.

PROGRAM ELIGIBILITY:

  • Over 18 years of age and live at home with your family

  • Served by the Regional Center

  • Family Assessment

    We come to your home and complete an assessment to learn about the individuals needs, strengths and barriers.

  • Receive Support

    We help with scheduling appointments, managing providers and services.
