Coordinated Family Supports (CFS)
CFS is a new program designed for adults who are 18 years and older who continue to live in their family home. Individuals must be served by a Regional Center.
CFS provides assistance and coordination of services and supports such as:
Developing skills in the home
Coordinating and getting services delivered, including help with “generic services”
Helping people access their community
Coordinating health/medical appointments
Assisting with scheduling and referrals across agencies
Help understand and navigate systems
Help secure other services such as respite, transportation and resources.

Over 18 years of age and live at home with your family
